In vitro fertilization, commonly called IVF, is a carefully coordinated fertility treatment in which eggs are collected from the ovaries, fertilized with sperm in a laboratory, and then one embryo is transferred into the uterus. For many individuals and couples, IVF can feel both hopeful and overwhelming. These medications encourage the ovaries to develop multiple mature eggs instead of the single egg typically produced in a natural cycle. The next day, the embryology team checks how many eggs fertilized normally. Over the following days, embryos are monitored as they divide and develop. Many clinics culture embryos to the blastocyst stage, usually day 5 or day 6. Not every egg becomes a mature egg, not every mature egg fertilizes, and not every fertilized egg develops into a usable embryo. This natural drop-off can be emotionally difficult, but it is a normal part of IVF biology.

Step 6: Genetic Testing and Embryo Freezing, If Recommended

Some patients choose preimplantation genetic testing, often called PGT. This involves taking a small number of cells from a blastocyst and freezing the embryo while results are pending. PGT may help identify embryos with the correct number of chromosomes, which can be especially relevant for patients of advanced maternal age, those with recurrent pregnancy loss, or couples at risk of passing on specific genetic conditions.

However, PGT is not necessary for everyone, and it does not guarantee pregnancy. Your doctor and genetic counselor, when appropriate, can help you understand the benefits, limitations, costs, and possible outcomes.

Step 7: Embryo Transfer

Embryo transfer is usually simpler and quicker than egg retrieval. It does not typically require anesthesia. A thin catheter is passed through the cervix, and the embryo is gently placed into the uterus. Ultrasound may be used to guide the placement. The procedure often feels similar to a Pap test, with mild pressure or cramping.

Your clinic may recommend transferring one embryo, especially if the embryo is high quality or genetically tested. Single embryo transfer reduces the risk of twins or higher-order multiple pregnancy, which can increase complications for both the pregnant patient and babies.

In a fresh transfer, the embryo is transferred a few days after egg retrieval. In a frozen transfer, embryos are frozen first and transferred in a later cycle after preparing the uterine lining. Frozen embryo transfer is now very common and may be recommended when hormone levels are high, the risk of ovarian hyperstimulation is increased, genetic testing is planned, or the uterine lining needs more controlled preparation.

Step 8: The Two-Week Wait and Pregnancy Test

After embryo transfer, you may take progesterone and sometimes estrogen to support the uterine lining. This medication can cause symptoms that resemble early pregnancy, such as breast tenderness, fatigue, bloating, and mild cramping. Because these symptoms can also be caused by hormones, they cannot reliably indicate whether the cycle worked.

The pregnancy test is a blood test measuring hCG, usually performed 9 to 14 days after transfer. Home pregnancy tests are not always reliable during this window, especially if a trigger shot was used recently. If the result is positive, your clinic will repeat bloodwork to confirm that hCG is rising appropriately and later schedule an ultrasound to confirm the pregnancy location and heartbeat.

If the test is negative, it is normal to feel grief, frustration, or numbness. A failed cycle does not necessarily mean IVF will never work. Your doctor will review the cycle response, egg maturity, fertilization, embryo quality, uterine factors, and transfer details to decide whether changes should be made for the next attempt.

Possible Risks and Side Effects

IVF is generally safe, but no medical treatment is risk-free. Side effects from medications can include bruising at injection sites, bloating, mood changes, headaches, and breast tenderness. Ovarian hyperstimulation syndrome, or OHSS, is an uncommon but important complication in which the ovaries become swollen and fluid shifts in the body. Modern protocols have reduced this risk significantly, but patients with polycystic ovary syndrome or very high ovarian response may need special precautions.

Other risks include bleeding, infection, or injury during egg retrieval, though these are rare. Multiple pregnancy is another major concern, which is why many clinics encourage single embryo transfer when appropriate. Ectopic pregnancy and miscarriage can still occur after IVF, just as they can occur with natural conception.

How to Prepare Yourself

Preparation can make IVF feel more manageable. Keep an organized medication schedule, ask your clinic what to do if a dose is late or missed, and write down questions before appointments. Maintain balanced meals, adequate hydration, and gentle movement as recommended by your doctor. Avoid smoking, limit alcohol, and discuss supplements or medications before using them.
